Gallstone Ileus
(R.A., f., 87 ani / years old)
 
 Clinical Manifestations: intermitent obstructive syndrome for 8 days.
Clinic: sindrom ocluziv de tip intestin subțire, evoluând intermitent de 8 zile.
Upright Abdominal Radiography: multiple enteric images with gas-fluid levels. Radioopaque calculus present in the right upper quadrant.
Radiografie abdominală simplă în poziție ortostatică: imagini hidroaerice multiple de tip enteric. În hipocondrul drept imagine de calcul radioopac.
Intraoperative Photograph: ( operation: enterolithotomy, enterorrhaphy, gallstone evacuation, duodenal fistula excision, duodenorrhaphy). Identification of the gallstone, which was impacted 30 cm proximal from the ileocecal valve.
Aspect intraoperator: (enterolitotomie cu enterorafie, colecistectomie parțială cu evacuarea calculilor, excizia fistulei duodenale, duodenorafie). Identificarea calculului biliar inclavat la cca 30 cm de valvula ileocecală.
Transversal enterotomy 3 cm proximal from the impacted stone.
Enterotomie transversală, la cca 3 cm aval de locul inclavării calculului.

 A - stone removal by digital extraction.
     B - enterorrhaphy.
A - extracția calculului prin presiune digitală. 
B - enterorafie.
Dissection of the hepatocholecystoduodenal block expose the scleroatrophic gallbladder, which contains 2 gallstones similar to the extracted  stone.
După disecția unui bloc hepatocolecistoduodenal, se identifică colecistul scleroatrofic mulat pe doi calculi asemănători celui extras din intestin.
Cholecystolitothomy.

 The scleroatrophic cholecyst and the duodenal fistula.
Colecistul scleroatrofic restant și fistula duodenală.
 After partial cholecystectomy and cholecystorrhaphy, transversal suture of the duodenal fistula.
După colecistectomie parțială (fundocorporeală) și colecistorafie, sutura fistulei duodenale, transversal.
 The calculus extracted through the enteric incision and one of the gallstones from the gallbladder.
Calculul extras prin enterotomie și unul dintre calculii din colecist.
